The VA Salt Lake City Health Care System (VASLCHCS) is made up of compassionate employees committed to caring for Veterans whose sacrifices know no limit. We are part of the Rocky Mountain Veterans Integrated Service Network (VISN 19). Our health care system consists of the George E. Whalen Utah, Idaho, and Nevada. We provide health care to over 50,000 eligible Veterans across one of the largest geographical areas in VA. We are a teaching hospital, providing full range of patient care services, holistic medicine as well as education and research in partnership with the University of Utah and other institutions of higher education. Comprehensive health care is provided through our primary care providers and tertiary care is offered in areas of medicine, surgery, psychiatry, physical medicine and rehabilitation, neurology, oncology, dentistry, and geriatrics.

Duties

The Nurse Practitioner for HBPC is responsible for managing an assigned group (panel) of patients with acute and chronic disease in the home setting. Management is achieved through assessment, planning, implementation, and evaluation to meet the needs of patients, in collaboration with the HBPC interdisciplinary team. The nurse practitioner promotes self-care and advanced illness planning and enlists the participation of patients in establishing realistic health goals. In addition to providing direct patient care, the nurse practitioner also provides patient and family teaching; staff education; consults and collaborates with other health providers, participates in nursing research; prescribes medications and adjunctive modalities; and refers patients. The HBPC NP adheres to the Specialty PACT Model. The nurse practitioner adheres to the American Nurses' Association Standards of Practice for the Primary Care/Family/Adult/Gerontological Nurse Practitioner. Duties include but not limited to:

  • Obtains and documents relevant health and medical history.
  • Documents in Computerized Patient Record System.
  • Conducts preventive screening procedures based on history.
  • Performs a physical examination based on the history.
  • Orders and interprets diagnostic tests.
  • Identifies medical and health risks and needs of the patient/family based on data collection.
  • Formulates the appropriate differential diagnosis based on the history, physical examination and clinical findings.
  • Establishes a plan of care to achieve the identified medical and health goals.
  • Prescribes (and administers) medications which may include controlled substances per licensure.
  • Identifies patient education needs and provides for relevant education.
  • Initiates referrals and/or consultations as appropriate.
  • Determines effectiveness of the plan of care through documentation of client (patient) care outcomes.
  • Is proactive in identifying potential emergency/crisis situations and providing appropriate interventions.
  • Performs procedures within the approved scope of practice and based on documented competencies such as excising skin lesions, administration of local anesthesia,, suturing, injection of joints, pelvic exam and pap smear, punch biopsy, etc.
  • Coordinates admission of outpatients to inpatient service and collaborates with physician when indicated.
  • Collaborates with the HBPC Interdisciplinary Team in delivering comprehensive care to veterans with complex medical and psychosocial problems and functional impairments.
  • Integral member of an interdisciplinary team providing healthcare services to veterans in the home to help them maintain their independence in the community.
  • Primary care provider for an assigned panel of patients.
  • Provision of primary care in the home for veterans in HBPC.
  • Identification of veterans' medical problems and development of the medical and nursing components of the interdisciplinary plan of care.
  • Prescription of medications and treatment in accordance with their individual scope of practice.
  • Determination of the need for consultation from subspecialty clinics and admission to the hospital.Participate in quality monitoring and team projects as assigned.
