    Responsibilities

    The Clinical Pharmacist is assigned to an outpatient behavioral health clinic and is responsible for medication therapy monitoring.  The incumbent delivers care to a specialty care patient population, similar to a primary care clinic pharmacist, makes recommendations to providers, performs medication order processing and routine assessment of medication therapy, and conducts patient counseling.  The pharmacist performs discharge medication reconciliation as an auxiliary duty during leave absences of the Behavioral Health Clinical Pharmacy Specialist.

    Duties include but are not limited to:  Handling routine medication-related activities in accordance with local, Veterans Integrated Service Network (VISN) and national policies and regulations.  The clinical pharmacist is the medication expert in all assigned areas, functions as a consultant to other health care professionals, and manages medication therapy across the continuum of care.  Plans, implements, and operates drug monitoring and utilization programs as assigned.

    Work Schedule: Monday - Friday; 8:00 am to 4:30 pm
