    Responsibilities

    The Mental Health Clinical Pharmacist Specialist for the acute inpatient psychiatry unit and the Substance Abuse Residential Rehabilitation Treatment Program (SARRTP) must be a licensed pharmacist with a Pharm.D. or with comparative experience, preferably with an accredited residency in psychiatric pharmacy, or equivalent experience demonstrating competency.  The clinical pharmacist is responsible for planning, implementing and operating the drug monitoring and utilization programs on the inpatient psychiatry unit and SARRTP. The clinical pharmacist uses expertise in mental health disorders symptom management and psychopharmacology to recommend optimized patient care, and functions as a consultant to other health care professionals.
    - Manages all aspects of pharmacy-related activities on the inpatient psychiatry unit and SARRTP, including order verification, discharge and Safe Medication Management (SMM) counseling, policy development, and specialty functions unique to the position to optimize patient outcomes.
    - Participate in multidisciplinary team rounds, providing drug information, observations on patient response to therapy, medication histories, and appropriate recommendations regarding treatment alternatives or additional interventions to maximize patient care outcomes taking into consideration choice of therapy, safety, efficacy, and pharmacoeconomics.
    - Interviews and/or evaluates patients upon admission to the hospital and daily during hospitalization for appropriateness of pharmacologic therapy. Documents all pharmaceutical care activities appropriately.
    - Provides consultative services to all professional staff regarding drug therapy.
    - Designs and/or participates in research studies or outcomes research involving drug therapy or clinical pharmacy functions.
    - Serves as a preceptor when appropriate for the students and residents at the VA San Diego Healthcare System. Evaluates residents and students based on performance, presentations and examinations. Maintains an organized, structured, clinical mental health rotation for pharmacy students and residents.
    - Participates in the development and implementation of formal and informal drug education/clinical consultation to medical, surgical, nursing, and pharmacy staff in the form of inservices and continuing education programs.
    - Creates an environment to stimulate, encourage, and support the participation of pharmacy personnel in providing concise, applicable, and timely responses to requests for mental health drug information to the medical center staff and other facilities as deemed appropriate.
    - Provides disease state or drug education, including safe storage of medication, to patients, family members or care givers to increase patient adherence to prescribed drug regimens.
    - Acts as a liaison between the Mental Health Care Line and the Inpatient and Outpatient pharmacies.
