    The Alexandria VA Health Care System is seeking a Clinical Pharmacist Specialist to provide service to the Alexandria VA Health Care System at the Fort Polk Community Based Outpatient Clinic. The Alexandria VA Health Care System, Alexandria, Louisiana is a Joint Commission accredited, complexity level 3 facility serving Veterans within 23 of Louisiana's 64 parishes. The VAMC is located in Pineville, Louisiana with community based outpatient clinics (CBOCs) in Lafayette, Jennings, Lafayette, Fort Polk, Lake Charles and Natchitoches, Louisiana.

    Responsibilities

    This position for a Clinical  Pharmacist Specialist with clinical duties providing pharmaceutical care to outpatients at the Fort Polk Community Based Outpatient Clinic, in Fort Polk, Louisiana. This position is located at the Fort Polk Community Based Outpatient Clinic (CBOC). He or she performs all duties assigned to a staff pharmacist relating to verification of prescriptions, filling, and medication counseling when necessary. The incumbent reviews the patient's drug regimen for appropriate indication, dosing, allergy verification, interactions potential, lab monitoring and non-VA medication documentation.   He or she provides pharmaceutical care services according to the scope of practice for clinical pharmacy specialists designated to specialty clinics for Hypertension, Anticoagulation, and Lipids as well as any additional clinics so described.  Incumbent provide cost-effective management of drug utilization within the above stated clinics and support the goals and objectives of the Health Care System cost reduction standards.  The incumbent develops protocols or clinical pathways to identify therapeutic goals for patients enrolled into these clinics.  He or she reviews patient's drug regimens for appropriate indication, dose, drug allergies, and duplication if medications and drug interactions.  He or she monitors the process and outcome of patient care relating to drug therapy.
