    Responsibilities

    This position will be in support of the Patient Aligned Care Team (PACT). The Clinical Pharmacy Specialist (CPS) functions at the highest level of clinical practice as mid-level practitioners, working independently under a global scope of practice as defined by the individual Medical Center to directly care for patients. The incumbent has prescriptive authority and is a mid-level provider that can initiate, modify and discontinue medications in accordance with their prescriptive privileges. A CPS is an expert in all assigned areas of coverage and functions as a consultant to other health care professionals.

    Major duties include, but are not limited to, the following:

    • Designs, implements, assesses, monitors and documents therapeutic plans utilizing the most effective, least toxic and most economical medication treatments.
    • Meets workload requirements (30-40 patient encounters per week) consistent with parameters outlined within the funding document.
    • Helps achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas.
    • Performs physical assessments.
    • Orders laboratory/other tests to help determine efficacy and toxicity of medication therapy.
    • Identifies, interprets, and resolves prescribing and dispensing issues, including pharmacotherapeutics and the formulary issues.
    • Identifies and independently resolves clinical and administrative problems that occur in their service area.
    • Effectively communicates across service lines as necessary to achieve resolution.
    • Implements strategies to optimize the cost-effective use of medications for KCVAMC.
    • Maintains working knowledge of applicable medication criteria for direct appropriate medication therapy and adjudicate non-formulary medication requests according to scope of practice.
    • Participates in the decision making process and willingly complies with team and leadership decisions.
    • Evaluates procedures/processes and demonstrates the ability to make timely and effective decisions that facilitate change and produce results that improve performance or patient care.
    • Assesses the educational needs of medical center staff and provides appropriate and timely formal/informal education on an ongoing basis.
    • Designs and coordinates drug utilization evaluations and the clinic's quality assurance program to include the collection and compilation of data and the preparation of reports to be provided to the appropriate committees.

    Work Schedule: Monday - Friday, 8:00 am - 4:30 pm
