    VA TEXAS VALLEY COASTAL BEND HEALTH CARE SYSTEM


    The VATVCBHCS is comprised of the Corpus Christi Outpatient Clinic (CCOPC), Corpus Christi Specialty Clinic (CCSC), Harlingen Outpatient Clinic (HOPC), Health Care Center at Harlingen (HCC), Laredo Outpatient Clinic (LOPC), and McAllen Outpatient Clinic (MCOPC). The VATVCBHCS is an ambulatory health care system providing health care in medicine, surgery, psychiatry, and rehabilitation medicine. The VATVCBHCS also operates a 100-patient Home Based Primary Care program and a 200-patient Care Coordination Home Tele-Health Program. Through the utilization of inpatient/emergency department contracts, approximately 2600 inpatient admission and 5800 ER visits are managed by VATVCBHCS. Affiliated with the University of Texas Regional Academic Health Campus, the system has an active ambulatory care program and serves as a teaching and training site for medical residents. The VATVCBHCS is responsible for an enrollee population of over 30,000 veterans and 350,000 outpatient visits annually.

    Responsibilities

    The Primary Care Mental Health Integration (PCMHI) Clinical Pharmacy Specialist serves as the liaison between Primary Care and Mental Health for Primary Care patients in the PACT model.  The PCMHI Clinical Pharmacy Specialist services the Mental Health medication therapy management needs for patients in the PACT model with less complicated Mental Health Service needs.  The incumbent acts as the primary specialist who functions as the custodian, distributor and consultant for all pharmaceutical agents and adjuncts, including their national uses and appropriate administration serving the Mental Health needs within Primary Care, serving those patients that are more efficiently served in the Primary Care setting.  The incumbent functions within the Corpus Christi Outpatient Clinic (CCOPC) as a Clinical Pharmacy Specialist and supports the Primary Care section as a clinical specialist.  Acts as a mid-level provider with prescriptive authority granted through an applicable Scope of Practice in the area assigned.

    Functions or Scope of Assigned Duties include, but are not limited to, the following: 

                       ·        Clinical Specialty Practice-Patient Care:

    o    Interfaces with Psychiatric care physicians in the design of the veteran's treatment regimen.

    o    Independently works in the assigned clinic area as a mid-level provider designing medication therapy management for Mental Health patients.

    o    Serves as an authoritative information source on drugs and their utilization in therapy.

    o    Designs dosage regimens for current pharmacokinetics principles and specific patient parameters. 

    o    Provides patient consultation concerning Mental Health drug therapy and related problems and needs.

                      ·        Dispensing:

    o    Interprets/evaluates physician orders for accuracy/appropriateness.

    o    Verifies physician medication orders and processes prescriptions.

    o    Evaluates and administratively completes non-formulary drug requests from providers in a timely manner, i.e., within the 96 hour requirement designated by Central Office.

    o    Prepares and dispenses medications in accordance with dosage form requirement, including routing outpatient prescriptions, compounded preparations and sterile products (including Rx admixtures). 

    ·        Education and Training Functions:

    o    Assists in the training of new pharmacy staff.

    o    Participates in in-service training programs for house staff.

    o    Serves as a preceptor to pharmacy residents and  students of Affiliated Schools of Pharmacy.  

    Work Schedule: 8:00 – 4:30 PM (Alternate schedules may be necessary to meet the needs of the organization)
