    Summary

    The Crescenz VA Medical Center - Philadelphia is a complex 1b tertiary care facility consisting of acute inpatient beds and long term care beds.  The organization has robust education and research programs and offers treatment for 40 different American Medical Associations recognized medical specialties and sub-specialties including Medicine and Surgery, Intensive Care, General Psychiatry, Substance Abuse Treatment, Long Term Care (Nursing Home Care Units) and Residential Rehabilitation Inpatient Program. The medical center is a teaching facility and is affiliated with the University of Pennsylvania School of Medicine and over 50 other colleges and universities.  The organization consists of the main Medical Center, three Community Based Outpatient Clinics, one clinical and administrative annex, a Community Living Center and a 40 bed Mental Health Residential Rehabilitation Program.

    Responsibilities

    Responsibilities:
    The primary responsibility of the Behavioral Health Clinical Pharmacy Specialist is the provision of evidence-based pharmaceutical care through direct, patient-centered care for Veterans and in consultation with other providers on their team.

    This incumbent acts as a representative of the Pharmacy Service and works independently, as an advanced practice provider with a scope of practice, to safely provide effective pharmaceutical care for our enrolled Veterans. They are responsible for promoting and managing medication therapy based on current clinical knowledge consistent with polices and guidelines established at the National, VISN and local levels.

    The Clinical Pharmacy Specialist must posses advanced knowledge and proficiency of clinical Pharmacy practice. Demonstrated expertise in Pharmacy operations and documented competency in Behavioral Health Pharmacotherapy is also required. The incumbent will provide primary clinical Pharmacy services and oversight for the facility's 40-bed Residential Rehabilitation Treatment Program (RRTP) and have the opportunity to work in expanding clinical Pharmacy services within the Medical Center's Outpatient Behavioral Health Clinics.

    Major Duties:
    Clinical:

    • Provides appropriate selection of drug therapy based upon the pharmaceutical principles of pharmacokinetics and pharmacodynamics; monitors for effectiveness, side effects, and clinical outcomes; and advises prescribers as appropriate.
    • Evaluates drug therapy through direct patient care involvement, with clinical assessment and physical measurement, to ensure a safe and effective clinical response and communicates both orally and in writing.
    • Performs continuous evaluation of prescribed medications to ensure optimal drug therapy.
    • Conducts comprehensive appraisals of patients' health status by taking health and medication histories, including patient interviews, and documents relevant findings in the patient's medical record.
    • Orders, performs, reviews, and analyzes appropriate laboratory tests and other diagnostic studies necessary to monitor and support the patient's drug therapy.
    • Follows-up with patients on laboratory and/or other test results to discuss the plan of therapy (i.e., changes in medication therapy, monitoring and evaluation, additional testing requirements, referral to primary care or specialty care provider).
    • Prescribes medications, devices, and supplies to include initiation, continuation, discontinuation, monitoring and altering therapy, based upon established formulary and/ or protocols.
    • Provides medication counseling to patients and teaches patients about drug and disease management.

    Drug Information:

    • Maintains a current knowledge of therapeutics and disease management.
    • Has a good working knowledge of drug information resources, both written and electronic.
    • Serves as a drug information resource by providing accurate, timely, and up-to-date drug information to medical and nursing staff, technicians, students, patients and caregivers.
    • Uses appropriate references to research and respond to drug information inquiries from all customers.

    Education and Research:

    • Skills at using both written and verbal communication techniques in teaching others, including the ability to adjust communication based on age and developmental considerations.
    • Keeps abreast of new developments in pharmacy practice and drug therapy by reading pharmacy and medical literature and participating in continuing education programs.
    • Assumes an active role in the overall pharmacy-training and inservicing program consisting of pharmacists, pharmacy technicians, pharmacy students, and pharmacy residents. Assists with new employee orientation sessions and conducts on-the-job training.
    • Provides education regarding medication use to patients and caregivers and accurately assesses and documents comprehension.
    • Provides educational in-services to nursing, medical staff or to other ancillary services as required.

    Dispensing:

    • Ensures all medication orders are entered into the patient's electronic medical record.
    • Reviews all medication orders for appropriateness, drug selection, dosage, route of administration and quantity prior to dispensing.
    • Processes and fills all orders in a timely fashion; review of the patient's allergy history, drug-drug, drug-nutrient, drug-laboratory and drug-disease state interactions and assessment for clinical relevance are performed and dealt with appropriately.
    • Reviews medication profiles and resolves all medication order problems encountered.
    • Selects, compounds and dispenses a range of drugs to be used in areas supported by both inpatient and outpatient operations.
    • Supervises medication repackaging and operation of automated dispensing equipment.

    Work Schedule: 7:30 AM to 4:30 PM, Monday - Friday.
