    VA Connecticut Healthcare System was officially formed in 1995. We serve veterans in Connecticut and southern New England. The West Haven facility is a tertiary care facility classified as a Clinical Referral Level I Facility. It is a teaching hospital, providing a full range of patient care services, with state-of-the-art technology and a renowned research program. Comprehensive health care is provided through primary, acute, tertiary and long-term care in areas of medicine, surgery, psychiatry, physical medicine and rehabilitation, neurology, oncology, dentistry, geriatrics and extended care. Newington is an ambulatory care center that provides primary and specialty care. In addition to our main campuses, we operate six community-based primary care centers throughout the state.

    The Incumbent will interface with a multi-disciplinary team of professional with in the Eastern Blind Rehabilitation Service, including disabled staff, blinded Veterans and staff or Veterans with Service animals. The incumbent must be able to work in work directly with patients in a courteous and professional manner. This combined emphasis on prioritizing and organizing has a significant impact on the work-flow process for these two functions within the service which results in increased applications and census for the Blind Rehabilitation programs, as well as increased patient satisfaction. Duties include but are not limited to:

    · Observe Blind Rehabilitation Service (BRS) students who are practicing class assignments, correct if needed, and report progress back tory Blind Rehabilitation Specialist, e.g., low vision, living skills

    · Provide skill reinforcement of Veterans during inpatient rehabilitation training, such as providing visual description, food preparation and meal time supervision, travel to the canteen, and other blind rehabilitation skills.

    · Administer national and/or in-house BRS patient satisfaction surveys and other discharge surveys as needed, to BRS patient; log and compile results.

    · Answer phones and responds to non-technical requests or forwards to staff for appropriate follow-up.

    · As assigned, escort/drives Veterans and Veterans with service animals upon admission/discharge or as needed. Functions as a driver for visually impaired staff as needed.

    · As assigned, escorts Veterans and Veterans with service animals using human guide3 as instructed to and from appointments.

    · Compiles data from the VSSC website for performance improvement projects and preparation for monthly performance improvement meetings.

    · Ensure compliance with Joint Commission, CARF, OSHA or other safety related requirements on removing expired items in the teaching kitchens and dayrooms.

    · Pick up orders from prosthetics for all EBRS staff; as needed, mail and ship prosthetics to Veterans.

    · Maintain, organize and update EBRS library of adaptive media for use by students and staff.

    · Modify spreadsheets to summarize data relevant to the Blind Rehabilitation Continuum of Care, both the inpatient and the low vision programs. Ensures information is in accessible formatting.

    The Incumbent will provide reasonable accommodation to disabled staff include:

    · Complete forms that not computerized or forms that computer screen reader software are unable to read properly, such as a Library of Congress or Bible application or shipping forms.

    · Label and identify items in kitchen and classroom, including label in large print and uncontracted Braille.

    · Type and place commonly used recipes in an accessible format (print and Braille) and saved onto the BRS common folder so they could be accessible to all instructors.

    · Serve a reader for visually impaired staff by reading reports, identify and read important information as identified by EBRS referral reports, community medical reports and outside service provider notes, especially handwritten documentation.

    · Read/proof students' handwriting assignments.

    · Organize and label in Braille, audio and print material, such as student schedule and instructional tapes.

    · Read and record hospital mandatory training or other hospital education programs which do not have all printed materials available in translatable formats.

    · Modify formats of a variety of software data formats to include; PDF, Excel, PowerPoint and formats not accessible using JAWS, Zoomtext or Voiceover.

    · Incumbent will perform other directly related duties, as assigned/ required.
