    Eastern Kansas Health Care System is a complexity 1c facility and has robust inpatient and outpatient treatment programs including acute medical inpatient, icu/pcu, acute psychiatry/ mental health rehab beds and three community living centers. We offer comprehensive outpatient physical medicine and rehab, mental health, geriatrics and prosthetics. We do extensive community outreach and placement.
    The Home Based Primary Care Mental Health Provider position can be filled with a Psychologist, Social Worker or Psychiatric Certified Nurse Practitioner.
    Individual Discipline Salary ranges
    GS-180 Psychologist: $61,605 - $114,146
    GS-185 Social Worker: $73840 - $95,991
    VN-610 Psychiatric Certified Nurse Practitioner: $87,431 - $127,910

    Area of Consideration:
    Leavenworth: First area of consideration will be given to NFEE bargaining unit employees at Leavenworth VAMC. Second area of consideration will be given to EKHCS permanent employees. Third AOC will be given to the public (U.S. citizens and U.S. nationals).
    Topeka: First area of consideration (AOC) will be given to NAGE permanent employees, Topeka VAMC. Second AOC will be given to any additional permanent employees of EKHCS. Third AOC will be given to the public (U.S. citizens and U.S. nationals).

    Responsibilities

    The Home Based Primary Care (HBPC) Mental Health Advanced Nurse Practitioner independently provides advanced practice clinical assessments, diagnosis, and treatment to Veterans enrolled in HBPC and is the primary mental health treatment provider within the program. Veterans served are enrolled in HBPC services and primarily receive their mental health care in the home setting. Their health care and psychosocial problems and needs are complex and require a high degree of clinical oversight, evidence-based treatment modalities and creative problem solving.
    Major duties will include, but are not limited to:

    • Patient Screening and Assessment.
    • Plans, provides, and evaluates care that promotes wellness and prevents disease/injury and disability.
    • Conducts and documents patient findings.
    • Based on mental health assessment, makes a psychosocial and psychiatric diagnosis within the scope of practice.
    • Plans effective treatment in coordination with Veterans and interdisciplinary teams.
    • Implements treatment.
    Work Schedule: Monday - Friday, 8:00 a.m. - 4:30 p.m.

    75% or less - Up to 75% travel to patient homes to provide care to patients.
