    The VA Southern Oregon Rehabilitation Center and Clinics (VA SORCC) in White City, Oregon is seeking a Health Care for Homeless Veterans HUD-VASH Program Senior Social Worker for our Klamath Falls CBOC clinic. The Department of Housing and Urban Development-VA Supported Housing (HUD/VASH) Program, through a cooperative partnership, provides long-term case management, supportive services and permanent housing support for homeless Veterans experiencing homelessness. The role of the HUD-VASH Case Manager is to provide eligible Veterans with case management and supportive services in order to support stability and recovery from physical and mental health, substance use, and functional concerns contributing to or resulting from homelessness. The incumbent is responsible for the delivery of such care in addition to collecting programmatic data, conducting community outreach aimed at identifying homeless and at-risk Veterans as well as building and sustaining successful community partnerships, and providing supportive services to eligible Veterans.

    The VA SORCC provides a variety of clinical programs with a wide range of outpatient services and treatments for Veterans in Geriatrics and Extended Care, mental health and substance abuse outpatient treatment, Psychiatry, substance abuse, home based primary care, community care, co-occurring Residential Treatment Programming,residential, and various other ambulatory care programs. The HUD-VASH Program is part of the outpatient Homeless Programs continuum at VA SORCC. Other Homeless Programs include: the Grant & Per Diem Program, Health Care for Homeless Veterans (HCHV) Program, and Veterans Justice Outreach (VJO) Program. The Homeless Programs teams consist of a variety of professionals including licensed Social Workers, Social Work Coordinators, Registered Nurses, Licensed Professional Counselors, Peer Support Specialists, and administrative staff.

    Work Schedule: Monday - Friday, 8:00am to 4:30pm

    Senior Social Worker. Senior Social Workers are licensed or certified to independently practice social work at an advanced level. Senior Social Workers typically practice in a major specialty treatment program area, such as a Polytrauma Rehabilitation Center or Polytrauma Network Site; a Spinal Cord Injury Rehabilitation Center, or a national VHA referral center, such as a national Center for Post-Traumatic Stress Disorder or a national Transplant Center, or other program areas of equivalent scope and complexity. The Senior Social Worker is assigned administrative responsibility for clinical program development and is accountable for clinical program effectiveness and modification of service patterns. They are also assigned in settings where they have no access to social work supervision, such as CBOCs or satellite outpatient clinics. They may be assigned to work with special patient populations with highly-complex health or mental health problems requiring expert level psychosocial interventions, such as VA/DOD liaisons assigned to Military Treatment Facilities as part of seamless transition or social worker case managers in Mental Health Intensive Case Management (MHICM) programs.
