The VA Salt Lake City Health Care System (VASLCHCS) is made up of compassionate employees committed to caring for Veterans whose sacrifices know no limit. Wahlen Department of Veteran Affairs Medical Center in Salt Lake City, Utah, and nine community clinics across Utah, Idaho, and Nevada. We provide health care to over 50,000 eligible Veterans across one of the largest geographical areas in VA. Duties

The Licensed Practical Nurse in the community-based outpatient clinic (CBOC) is an integral part of the Patient Aligned Care Team (PACT). The PACT LPN works under the direction of an RN and reports to the CBOC manager. The LPN is responsible for observing, recording and reporting changes in the patient's condition, as well as assisting in the development and revision of the patient-driven holistic care plan for life. The LPN requires a general review of their work by an RN for appropriateness and conformity with license, policies and procedures. The LPN demonstrates active collaboration and communication with Teamlet (Patient, Primary Care Provider, RN Care Manager and Clinical Support, Medical Support Assistant (MSA) and Larger Team (family/caregiver, internal and community based services and disciplines involved in providing care to the patient).

RESPONSIBILITIES:
1. Makes observations and thoroughly documents care and the patient's response. Appropriately seeks assistance and guidance by informing RN or medical provider of changes in the patient's condition requiring higher-level intervention. The LPN is responsible for observing, recording, and reporting changes in the patient's condition, as well as assisting in the development and revision of the patient-driven holistic care plan for life.
2. The LPN possesses the knowledge and skills to perform, report, and record assigned nursing care in the Primary Care Clinics, including, but not limited to:
a. Oxygen therapy
b. Medication administration to include IV fluids [if certified]
c. Phlebotomy performance and specimen collection
d. Focuses on clinic visits, exams and treatments with provider
e. Uses motivational interviewing and health coaching to guide patients in order to promote patient self-management toward individualized life plan of care.
f. Reviews daily schedules for patient care needs
g. Attends and contributes to Team huddles
h. Assists with triage: messages from patients and face -to-face visits
i. Processing Clinical reminders with appropriate consults and alerts as indicated
j. Pre- visit contacts
k. Check expiration dates of meds and supplies
l. Delegated procedures and treatments
m. Documentation
n. Assists with coordination of group visits
o. Assists with orientation: PACT patient orientation and new staff.
p. Assists with minor procedures, including obtaining and arranging necessary equipment, positioning patient, ensuring comfort and privacy. Monitors patient during the procedure.
3. Adheres to standards of practice and safe technique in preparation and administration of medication.
4. Adheres to Laboratory standards of practice in phlebotomy performance, specimen collection, preparation and packaging for transport.
5. Demonstrates understanding of age- and population-specific characteristics that affect the health care needs of the Veteran.
6. Promptly recognizes urgent or emergent patient care situations, seeks assistance and effectively intervenes as directed when appropriate.
7. Based on the patient?s goals, provides patient and family health education, to include self-management, prevention and wellness.
8. Interacts and communicates appropriately with patients and family members.
9. Demonstrates professional behavior (i.e., respect, civility, ability to accept constructive criticism), effective communication and interpersonal skills.
10. Adheres to principles of infection control, safety, and universal precautions.
11. Adheres to all requirements in regard to reusable medical equipment.
12. Responsible for minor adjustments of the PCP schedule prior to daily huddles i.e, labs, appointments, hospital/ER discharges, staff shortage.
13. Demonstrates competence utilizing special equipment needed to perform job duties.
14. Follows policy and procedures related to fire and disaster drills, safety and security measures.
15. Demonstrates initiative in assisting the Teamlet and larger Teams.
16. Assists in the orientation and competency validation of students, peers and clerical staff.
17. Maintains confidentiality of electronic, written, and/or verbal patient information.
18. Maintains a work environment that honors diversity.
19. Serves as a resource and advocate in the protection of patient and family rights.
20. Completes mandatory reviews, including motivational interviewing, in order to promote patient self management toward individualized life plan of care.
21. Supports patient-driven care in a safe, efficient and cost-effective manner, using evidence-based principles.
22. Promotes patient and personnel safety.
23. Serves as a support/mentor to less experienced LPNs.
24. Additional non Primary Care tasks will be assigned on an ongoing basis, based on skill set and availability.

Work Schedule: Monday-Friday; 8:00 a.m. to 4:30 p.m.
