The VATVCBHCS is comprised of the Corpus Christi Outpatient Clinic (CCOPC), Corpus Christi Specialty Clinic (CCSC), Harlingen Outpatient Clinic (HOPC), Health Care Center at Harlingen (HCC), Laredo Outpatient Clinic (LOPC), and McAllen Outpatient Clinic (MCOPC). The VATVCBHCS is an ambulatory health care system providing health care in medicine, surgery, psychiatry, and rehabilitation medicine. The VATVCBHCS also operates a 100-patient Home Based Primary Care program and a 200-patient Care Coordination Home Tele-Health Program. Through the utilization of inpatient/emergency department contracts, approximately 2600 inpatient admission and 5800 ER visits are managed by VATVCBHCS. Affiliated with the University of Texas Regional Academic Health Campus, the system has an active ambulatory care program and serves as a teaching and training site for medical residents. The VATVCBHCS is responsible for an enrollee population of over 30,000 veterans and 350,000 outpatient visits annually.

*** Bargaining Position ***

You will serve as the Licensed Professional Mental Health Counselor (Housing and Urban Development/VA Supportive Housing Case Manager) with the VA Texas Valley Coastal Bend Health Care System's (VATVCBHCS).  Through a cooperative partnership with the Department of Housing and Urban Development and the Department of Veterans Affairs Supportive Housing (HUD-VASH) Program, you will provide long-term case management and supportive services in order to support stability and recovery from physical and mental health, substance use, and functional concerns contributing to or resulting from homelessness. You will be responsible for the delivery of such care in addition to collecting programmatic data, conducting community outreach aimed at identifying homeless and at-risk Veterans as well as building and sustaining successful community partnerships, and providing supportive services to eligible Veterans. You will also be responsible for all aspects of the HUD-VASH program to include, but not limited to:  

  • Reviewing agreements between Section 8 landlords, Public Housing Authorities and the VA to establish appropriate referral sites for HUD-VASH placements;
  • Site visits of HUD-VASH apartment to ensure that Veterans reside in safe environments in compliance with local housing codes;
  • Admissions to the HUD-VASH program, ensuring that homeless Veterans are considered as primary participants for program acceptance;
  • Serve as an advocate on behalf of the Veterans to ensure comprehensive service delivery, and linking and referring Veterans to the VA Health Care System VA Regional Offices or community agencies. 

Work Schedule: Monday - Friday 8:00 a.m. – 4:30 p.m.(Alternate schedules may be necessary to meet the needs of the organization.)
