The Overton Brooks VA Medical Center (OBVAMC) consists of one VHA tertiary care facility and three (3) community based outpatient clinics serving Veterans in twelve (12) Louisiana parishes, eleven (11) counties in Southern Arkansas and nine (9) counties in East Texas. The medical center is classified as a Clinical Referral Level 1C Facility which accepts referrals from the Southeast Louisiana Veterans Healthcare System located in New Orleans, LA and the Alexandria VA Health Care System located in Pineville, LA. OBVAMC is a teaching hospital providing a complete range of patient care services, with state-of-the-art technology, as well as education and research. Comprehensive health care is provided through primary and specialty care in the areas of medicine, surgery, psychiatry, physical medicine and rehabilitation, neurology, oncology, dentistry, and geriatrics. The Midlevel Provider assigned to Primary Care Service is responsible to the Medical Director, Community Based Outpatient Clinic (CBOC), who is responsible to the Chief, Primary Care; provides quality patient care, based on the medical home model of care ; works collaboratively with other health care professionals to accomplish patient, service, hospital, and VISN goals.  He/she must display leadership and an exceptional degree of competence.  He/she will be involved with a wide variety of specialties/subspecialties. Duties include, but are not limited to the following:

. Performs and records history and physical examinations and records data from clinic visits.  Enters progress notes in computerized medical record and completes workload documentation in a timely manner. Processes patient alerts in a timely manner.

. Participates with all members of the Patient Aligned Care Team in caring for the patient to promote the medical home concept, including daily team huddles, weekly team meetings, and fostering the team concept.

. Orders diagnostic tests, medications, laboratory tests as medically relevant for each patient. Notifies patients of test results within allotted time frames, according to policy.

 . Determines medical assessment and develops treatment plans according to accepted protocols, physician supervisor recommendations or standing orders.

. Prescribes medications according to approved formulary, and in accordance with Federal and State Law.

 . Educates patients concerning medical conditions and recommended treatments.  (Specific topics include nutritional counseling, need for smoking cessation, need for periodic immunizations and health screening tests, and other preventive health measures).

. Consults other specialties as appropriate, according to service agreements

Work Schedule: Generally, Monday through Friday, 8:00 am - 4:30 pm.
