    Responsibilities

    The Nurse Practitioner position is part of the Mental Health Intensive Case Management (MHICM) treatment team in the Mental Health Service, at the VA Central CA Healthcare System (VACCHCS), Fresno, CA.

    The MHICM Nurse Practitioner functions autonomously as guided by functional statement/scope of practice and collaboratively with the multi-disciplinary Mental Health Intensive Case Management (MHICM) treatment team to manage Veterans diagnosed with a serious and persistent mental illness.The MHICM Nurse Practitioner provides community based care with the expectation of 80% of his/her time spent in the community with Veterans. Due to the intensity of the case management and severe co-morbidity of these veterans, the nurse case manager/veteran ratio is 1: 15 or less.

    Veteran care/needs is recovery focused with a common goal of obtaining maximum independence and re-integration into the community. The Nurse Practitioner collaborates with the Veteran and team members in the development of a strengths-based plan of care to assist Veteran in achieving his/her identified goals. The Nurse Practitioner provides recovery oriented care for Veterans utilizing their clinical expertise in individualized patient -centered treatment planning, symptom and functional assessment and skill training, education, research and consultation. The Nurse Practitioner implements evidence based education methods that promote Veterans informed collaboration rather than passive compliance. The Nurse Practitioner initiates and maintains a collaborative, supportive, mutually respectful therapeutic alliances, and is an advocate for the
    Veteran.

    The patient population includes Veterans who are likely to present with Severe Mental Illness (SMI), PTSD, mood disorders, substance abuse disorders, anxiety disorders and concomitant medical co- morbidities. The Nurse Practitioner is knowledgeable in assessment, treatment and evaluation of a comprehensive range of health care problems including but not limited to: preventive health, prescribing medications and other treatments; ordering, conducting and interpreting laboratory, diagnostic studies and procedures; and patient and family education utilizing community, social and health resources enhancing recovery principles. The Nurse Practitioner may be assigned a panel of patients to manage their health care, assigned urgent or acute care duties or assist with other specialized Mental Health Programs like Psychosocial Rehabilitation and Recovery Center (PRRC) as an example.

    Other responsibilities include:

    • Performs specialized treatment of a complex physical and/or mental illness.
    • Provide individual, group, and family interventions and case management interventions used in the treatment of Veterans with serious mental disorders.
    • Develops and participates in the development of a treatment plan for each MHICM
    • Veteran with measurable goals outlined based on relevant psychological, medical,
      and psychiatric information.
    • Offers consultation to colleagues, students and community care providers on the
      psychosocial treatment of Veterans.
    • Responds to consults sent to the MHICM program by interviewing the Veteran.
    • Assists MHICM Veterans in the preparation of materials needed for applications to
      various agencies for entitlements.
    • Identifies, develops and facilitates accessibility and utilization of VA and community
      resources.
    • Conducts home visits and community visits to MHICM Veterans.
    • Collaborates with Adult Care homes and nursing care facilities where MHICM
      Veterans reside.
    • Provides assistance with medication management.
    • Provides transportation of program Veterans in Government vehicles.

    Work Schedule: Full-time, Monday-Friday; 8:00am-4:30pm, some early evening hours when needed.
