    Responsibilities

    Clinical Responsibilities of Occupational Therapist
    The Occupational Therapist will provide services for this Medical Center, including the Community Based Outpatient Clinics. The Occupational Therapist works independently with occasional supervision from the Administrative Officer and Chief of Primary Care.
    Clinical Practice/Patient Care:
    Uses standard and non-standard methods and techniques for evaluating and interpreting a wide variety of physical and psychosocial functioning levels and provides progressive treatment.
    Develops interventions and a plan of care to include measurable short and long-term goals, frequency and duration of treatment interventions, discharge planning and consideration of both behavioral and functional performance.
    Devises or adapts equipment and employs a variety of motivational techniques to provide treatment to medically and/or emotionally complex patients.
    Provides interventions by utilizing purposeful activities to enhance occupational performance in the areas of daily living, work and leisure activities by addressing sensory/motor, cognitive and psychosocial skills.
    Conducts both group and one-to-one therapeutic interventions.
    Works collaboratively with other services such as physical therapy, Speech and Language Pathologists, psychiatry, and other primary care departments.
    Instruct patients by using practical demonstration
    Administrative/Liaison/Consultation/Collaboration:
    Serves as an occupational therapy consultant for nurses, physicians and other health care professionals.
    Identifies and collaborates in the resolution of system issues which hamper the provision of quality patient care.
    Participates in medical center committees or task forces to contribute to the improvement and or provision of healthcare services.
    Outcomes Management/Quality Improvement:
    Develops outcome measures to monitor the quality/outcomes of services.
    Incorporates research, analyzes data collected to improve processes.
    HOME BASED PRIMARY CARE
    This assignment carries primary responsibility for providing in-home occupational therapy assessments, and the planning, and administration and management of rehabilitation in the home for patients of the Sheridan VA Medical Center Home Based Primary Care Program (HBPC). The OT will provide these in-home services to patients with a wide variety of multiple, complex chronic illnesses and/or profound disabilities.
    Assessment and treatment of patients and training of students and various disciplines is independently conducted by the incumbent who exercises professional judgment regarding each patient Decisions are made in the community without onsite supervision of another health care professional.
    The Occupational Therapist, HBPC has a bachelor's degree from an accredited school of occupational therapy, and maintains a valid state occupational therapy license. It is necessary that the incumbent have a working knowledge of the applications of occupational therapy, physical therapy, speech therapy and recreational therapy, to ensure appropriate rehabilitative assessments, treatment, and management, whether directly or by referral to a community therapist.
    The Occupational Therapist, HBPC position is aligned within the Home Based Primary Care section of the Geriatrics &Extended Care Service, under the supervision of the Clinical Program Manager and in collaboration with the Medical Director, and/or Primary Care Provider, HBPC.

    Work Schedule: Monday - Friday 7:30am to 4:00pm
