Duties

This announcement is for a position as a Physical Therapist with the Home Based Primary Care (HBPC) Program for the Kansas City VA Medical Center, primarily assigned to the Excelsior Springs team, but to provide periodic Physical Therapy services for selected patients within HBPC as assigned.  HBPC is a unique model of home care that is very different in target population, process, and outcomes from home care available under Federal and State programs, such as Medicare and Medicaid. The HBPC model targets persons with complex chronic diseases that worsen over time and provides interdisciplinary care that is longitudinal and comprehensive rather than episodic and focused. HBPC provides cost effective primary care services in the home and includes palliative care, rehabilitation, disease management, and coordination of care. Members of the HBPC team are expected to visit veteran homes as part of their regular, recurring job duties. 

Major Duties of this position include, but are not limited to, the following:

**Functions as a collaborative rehabilitative health provider of the HBPC teams, providing rehabilitative health prevention, assessment, treatment, management, and professional consultation services in the veteran's residential setting coordinating with OT services.

**Independently develops, implements, and evaluates therapeutic rehabilitative programs within the framework of HBPC and Physical Medicine and Rehabilitative Service (PM&RS).

**Assesses goals, living skills, physical abilities, and related needs for each patient referred for in-home assessment.

**Develops treatment plans directed toward achieving the maximum level of functioning within the home, consulting with other PM&RS disciplines as indicated, and developing the plan in collaboration w ith the patient and caregiver.

**Implements treatment plans by providing direct patient care, procuring necessary equipment/adaptive devices, instructing and training client and caregiver concerning exercises, activities of daily living, rehabilitation techniques, and equipment/adaptive devices. Recommends community referrals when needed.

**Reassesses patients' levels of functioning and home environment, revising the treatment plan as indicated and terminating treatment objectives when met and/or the patient has achieved maximum benefit from PM&RS.

**Evaluates the home for structural or positional changes in order to promote the patient's safety and independence.

**Develops and coordinate educational materials for the caregiver of the veterans indicated by the PT assessment and Joint Commission home care standards.

**Participates in HBPC Performance Improvement Program, including completing assessment and documentation required by performance measures applicable to the PT's role.

**Serves as content expert in ordering complex and specialized medical equipment.

**Participates in interdisciplinary team conferences.

**Serves as consultant to other professionals in the evaluation of patient with multiple and complex problems in the home, for which highly specialized knowledge and skills are required.

Work Schedule: Monday - Friday, 8:00 am to 4:30 pm
