The VA Saint Louis Health Care System is seeking a Physician to lead a specialized high-intensity care team, the ComPACT, that is designed for veterans with complex medical conditions. The goal of the program is to improve the quality of life and reduce hospital admissions among these patients by providing them with intensive, highly coordinated multidisciplinary outpatient care and by improving transitions of care when they are admitted to and discharged from the hospital. The current pilot project has had a dramatic positive impact on the quality of life of our patients and has reduced the rate of admissions and ED visits among our patient group by about one half. We are the outpatient Primary Care Providers for our patients and we round on them on a daily basis when they are inpatients.
The physician will lead an interdisciplinary team that includes a mid-level provider, RN case manager, LPN, Pharmacist, Social Worker, Psychologist, and Dietician. The primary duties for the successful candidate include, but not limited to;

  • Provides medical and administrative oversight of ComPACT
  • Sees the ComPACT patients as the outpatient Primary Care Provider
  • Conducts intensive daily phone follow-up and telehealth review
  • Rounds and writes daily notes on the ComPACT inpatients
  • Collaborates in discharge planning with house staff and medicine attending and writes discharge instructions
  • Reviews consults of new referrals
  • Performs direct inpatient assessment of new consults if the patient is in the hospital
  •  Analyses ComPACT data

Work Schedule: Monday through Friday 7:30am to 4:00pm
