VA Connecticut Healthcare System was officially formed in 1995. We serve veterans in Connecticut and southern New England. The West Haven facility is a tertiary care facility classified as a Clinical Referral Level I Facility. It is a teaching hospital, providing a full range of patient care services, with state-of-the-art technology and a renowned research program. Comprehensive health care is provided through primary, acute, tertiary and long-term care in areas of medicine, surgery, psychiatry, physical medicine and rehabilitation, neurology, oncology, dentistry, geriatrics and extended care. Newington is an ambulatory care center that provides primary and specialty care. In addition to our main campuses, we operate six community-based primary care centers throughout the state.


The Medical Director for the Home and Community Based Program (HBPC) at VACT Health Care System is the senior clinical in charge of the program and is responsible for the overall medical care delivered by the HBPC team. The Medical Director reports directly to the Manager of Geriatrics & Extended Care.  The incumbent works in collaboration with the HBPC Program Director to meet al requirements of the HBPC program.  The incumbent is expected to qualify for a faculty appointment and be involved in academic activities with affiliated Medical Schools. The incumbent is also responsible for meeting home care accreditation standards, including, but not limited to VHA and The Joint Commission standards for home care.  Duties include but are not limited to:

· Develop and monitor the HBPC performance improvement plan

· Select HBPC team members

· Monitor appropriateness of care provided to patients enrolled in the HBPC Program

· Visit the HBPC patients at their home

· Arrange physician and LIP coverage and communicate the plan coverage to the HBPC team

· Provide direct assessment, evaluation, and care to HBPC enrolled veterans

· Plan and direct the educational and clinical experience of medical students, residents and fellows assigned to the HBPC programs;

· Advocate for HPBC with VHA leadership and the medical community;

· Assuming primary medical responsibility for all HBPC patents

· Works with an interdisciplinary team managing the care of chronically ill home-bound patients

· Provide care in collaboration with nurse practitioners on an innovative extended care unit with a mix of sub-acute, long term care, hospice/palliative care and respite patients

· Providing medical and administrative oversight including review and approval of documentation requiring physician's signature for the HBPC patients.

Work Schedule: Monday- Friday 7:30am to 4:00pm

  • Position requires regular travel to and from Veteran’s homes. Travel is also required to and from West Haven and Newington campuses and may require travel to CBOC’s as well.
