    The VA Salt Lake City Health Care System (VASLCHCS) is made up of compassionate employees committed to caring for Veterans whose sacrifices know no limit. We are part of the Rocky Mountain Veterans Integrated Service Network (VISN 19). Our health care system consists of the George E. Wahlen Department of Veteran Affairs Medical Center in Salt Lake City, Utah, and nine community clinics across Utah, Idaho, and Nevada. We provide health care to over 48,000 eligible Veterans across one of the largest geographical areas in VA. We are a teaching hospital, providing a full range of patient care services, holistic medicine as well as education and research in partnership with the University of Utah and other institutions of higher education. Comprehensive health care is provided through our primary care providers and tertiary care is offered in areas of medicine, surgery, psychiatry, physical medicine and rehabilitation, neurology, oncology, dentistry, geriatrics, and extended care.

    The Licensed Practical Nurse in the Primary Care Clinic is an integral part of the Patient Aligned Care Team (PACT). The PACT LPN works under the direction of an RN and reports to the primary care manager. The LPN is responsible for observing, recording and reporting changes in the patient's condition, as well as assisting in the development and revision of the patient-driven holistic care plan for life. The LPN is a member of the larger Team. The LPN requires a general review of their work by an RN for appropriateness and conformity with license, policies and procedures. The LPN demonstrates active collaboration and communication with Teamlet (Patient, Primary Care Provider, RN Care Manager and Clinical Support/ Patient Service Administrative Assistant (PSA) and Larger Team (family/caregiver, internal and community based services and disciplines involved in providing care to the patient).This position makes observations and thoroughly documents care and the patient's responses. The incumbent will seek assistance and guidance by informing RN or medical provider of changes in the patient's condition requiring higher-level intervention. The LPN is responsible for observing, recording, and reporting changes in the patient's condition, as well as assisting in the development and revision of the patient-driven holistic care plan for life.

    The LPN is required to possess the knowledge and skills to perform, report, and record assigned nursing care in the Primary Care Clinics, including, but not limited to:

    • Assists patients with Oxygen therapy
    • Administers medication to include IV medication if certified
    • Focuses on clinic visits, exams and treatments with provider
    • Uses motivational interviewing and health coaching to guide patients in order to promote patient self-management toward individualized life plan of care.
    • Reviews daily schedules for patient care needs
    • Assists with triage: messages from patients and face -to-face visits
    • Processing Clinical reminders with appropriate consults and alerts as indicated
    • Makes Pre-visit contacts
    • Check expiration dates of meds and supplies
    • Assists with orientation: PACT patient orientation and new staff.
    • Assists with minor procedures, including obtaining and arranging necessary equipment, positioning patient, ensuring comfort and privacy. Monitors patient during the procedure.
    • Adheres to standards of practice and safe technique in preparation and administration of medication.
    • Demonstrates understanding of age- and population-specific characteristics that affect the health care needs of the Veteran.
    • Promptly recognizes urgent or emergent patient care situations, seeks assistance and effectively intervenes as directed when appropriate.
    • Based on the patient's goals provides patient and family health education, to include self-management, prevention and wellness.
    • Interacts and communicates appropriately with patients and family members.
    • Demonstrates professional behavior (i.e., respect, civility, ability to accept constructive criticism), effective communication and interpersonal skills.
    • Adheres to principles of infection control, safety, and universal precautions.
    • Adheres to all requirements in regard to reusable medical equipment.
    • Responsible for minor adjustments of the PCP schedule prior to daily huddles (i.e., labs, appointments, hospital/ER discharges, staff shortage).
    • Assists in the orientation and competency validation of students, peers and clerical staff.
    • Serves as a resource and advocate in the protection of patient and family rights.
    • Completes mandatory reviews, including motivational interviewing, in order to promote patient self-management toward individualized life plan of care.
    • Supports patient-driven care in a safe, efficient and cost-effective manner, using evidence-based principles.
    • Promotes patient and personnel safety.
