    If you are looking for an exciting career challenge in a beautiful Colorado town the Grand Junction VHCS and surrounding areas have a lot to offer. The growing city has 60,000 residents making it the largest city in western Colorado and sits at 4,593 feet above sea level. Grand Junction is located on the Western Slope of Colorado where the weather is known to be mild throughout the year due to the lower altitude. Other well-known activities in the area are camping, hunting, hiking, mountain and road biking, rock climbing, golfing, rafting, fishing, skiing, and the vast amount of orchards and wineries. Grand Junction is known for the fresh peaches and other produce available, which in turn produce weekly farmer's market. Colorado Mesa University, a campus of the University of Colorado is located in Grand Junction as well. The Grand Valley is a destination for several athletic events including marathons, triathlons and mountain bike races. The annual Winefest highlights the nearly 30 wineries that make their home in Palisade, Colorado, also part of the Grand Valley. Grand Junction Veteran Health Care System (GJVHCS) is the home of Disabled Veterans Winter Sports Clinic and is a full service acute care facility with both inpatient and outpatient services. The facility maintains 24/7 acute care service including ICU and an emergency department.

    Responsibilities

    Major Duties: This psychologist will be a member of the VA Home-Based Primary Care (HBPC) program that provides comprehensive, interdisciplinary primary care services in the homes of veterans with complex, chronic, and disabling disease.
    Clinical Responsibilities (100% of time)
    The Provider functions as the primary mental health provider of the HBPC team, providing mental health prevention, assessment, treatment, management, and professional consultation services. Specific duties include:

    • Providing screening, assessment, diagnosis, and treatment of depressive and anxiety-related disorders, as well as other Axis I conditions, with an emphasis on the application of time-limited, evidence-based approaches;
    • Assessing and providing treatments for subclinical symptoms of dysphoria and anxiety, adjustment difficulties, and excessive use or misuse of alcohol or other drugs (for which the patient may have a lower than normal tolerance due to their medical conditions and the potential for negative interactions with prescribed medication);
    • Providing services to the families of Veterans with dementia being served by HBPC, when such care is directly related to the overall treatment plan for the veteran and will allow better care for the Veteran and allow the family to sustain the veteran in the home environment.
    • Providing clinical services through the use of Telemental health/telehome technology;
    • Providing screening for cognitive deficits to address specific functional questions or to determine if a referral for a complete neuropsychological evaluation or dementia assessment is needed;
    • Providing psycho-educational and supportive interventions, in conjunction with the veteran, for spouses, family members/caregivers, and others that are crucial to keeping the veteran at home;
    • Providing effective interventions with couples or families, when appropriate, to relieve relationship difficulties, provide support to caregivers of veterans with dementia or other significant frailty, and/or promote collaboration with the HBPC team, for the benefit of the Veteran.
    • Facilitating transition and adjustment to new living situations (e.g., nursing home, assisted living facility), and providing seamless intervention services, as appropriate;
    • Promoting communication/interactions between medical team members, patients, and their families to facilitate the medical treatment process;Consultation Services HBPC
    • Provides consultation to HBPC program staff, and primary care physicians, and mental health professionals in the Psychiatry Service concerning assessment findings and treatment plans for patients. This consultation is in the service of ensuring that care is coordinated, collaborative, and integrated within the provider team in the primary care setting.Administrative responsibilities
    • Completes all mandatory training requirements expected of Grand Junction VHCS employees.
    • Assists the HBPC Nurse Manager or coordinator in the design, development, and implementation of clinical programs for a team of treatment providers.
    • Assist the HBPC Nurse Manager or coordinator in the execution of ongoing program evaluation and data-driven quality improvement projects to monitor the effectiveness of the clinical program
    • Complies with national and local performance measures and clinical reminders, including timely health care screening and completion of clinical reminders for all patients.
    • Complies with all CPRS documentation procedures, including timeliness standards for documentation and proper patient check out procedures (e.g., entering diagnostic and procedure codes).
    • Attends and participates in administrative and informational meetings held by the Director of HBPC, Section Chief of the Psychotherapy and Psychosocial Services, Chief of the Psychiatry Service, Chief of Staff and Facility Director.
    • Attends weekly HBPC and Psychotherapy and Psychosocial Service staff meetings and complies with Psychiatry Service peer review procedures, and ongoing professional practice evaluations.
    Work Schedule: M-F, 0800-1630 (Subject to change as the HBPC Team grows)
