    The VA Salt Lake City Health Care System (VASLCHCS) is made up of compassionate employees committed to caring for Veterans whose sacrifices know no limit. We provide health care to over 50,000 eligible Veterans across one of the largest geographical areas in VA. We are a teaching hospital, providing full range of patient care services, holistic medicine as well as education and research in partnership with the University of Utah and other institutions of higher education. Comprehensive health care is provided through our primary care providers and tertiary care is offered in areas of medicine, surgery, psychiatry, physical medicine and rehabilitation, neurology, oncology, dentistry, and geriatrics.

    Responsibilities

    The Acute Medicine nurse is a professional who manages the care of the acutely ill patient with problems in medical, oncology, and rehab needs through collaboration with nursing staff, physicians and other health care providers. The nurse coordinates and facilitates care to restore stability, minimizing complications, and achieve optimal patient outcomes within a dynamic, acute care environment. In order to accomplish this, the professional nurse uses the Nursing Process in the facilitation and coordination of care for patients with a team of caregivers.
    The Acute Medicine staff nurse is a registered professional nurse who is accountable for the provision of patient care on Acute Medicine in both the inpatient and outpatient settings and is directly responsible to the Nurse Manager. He/she must demonstrate the ability to work effectively as a team member and must be able to communicate, listen, report and gather crucial data in a manner that enhances patient outcomes and utilize the computer charting system.
    The Acute Medicine Unit supports professional growth through education activities, attendance and participation on unit or hospital committees, Quality Improvement projects, or membership in professional organizations. Duties include but not limited to:

    • Completes admission assessment on assigned patients within 24 hours of admission
    • Reassessments patient condition every 12 hours or as needed.
    • Transcribes physicians' orders using CPRS
    • Documents vital signs and assessments using CPRS
    • Take verbal and telephone orders per policy
    • Completes patient care plans according to policy
    • Plans for discharge/transfer of patients including completing discharge/transfer note in CPRS
    • Documents medications using BCMA ( Bar Code Medication Administration)
    • Provides individualized patient care consistent with identified knowledge of patient's strengths and limitations
    • Is proactive in identifying potential crisis situations and responds appropriately in emergency/code situations
    • Initiates referrals and/or consultations as appropriate to achieve the multidisciplinary plan of care
    • Functions independently in performing more complex treatments, procedures and advanced technology specific to the scope of care on Acute Medicine according to the Procedure Manual (ie chemotherapy administration, pain management/comfort care and rehab.)
    • Recovers post procedure patients (ie heart caths, vascular, G.I., Renal, angios)
    • Performs blood draws and venipuncture for IV therapies, IV push medications, TPN and blood products , and Chemotherapy according to policy using a variety of access devices
    • Sets up and monitors a variety of pumps, including PCA
    • Promotes and provides health care teaching to patient and significant others
    • May Rotate into outpatient areas (ie clinics, AMU, heart cath, 23 hr stays)
    • Functions competently as modular leader ( Nurse I, level I) and/or charge nurse (Nurse I, level 3) must be able to function in charge nurse role)
    • Listens to report for entire module
    • Adjusts assignments based on patient acuities and new admissions
    • Coordinates coverage for SNT/LPN/CNA
    • Collaborates with team members, other disciplines and patient/significant others
    • Recognizes and uses the knowledge and expertise of colleagues and other health care providers in meeting patient care needs

    Work Schedule: Rotating to include nights, weekends, and holidays.
