    Responsibilities

    The RN Care Coordinator uses the nursing process and evidence-based practice to collaborate with the PACT team members (patient, Provider, Medical Support Assistant and LPN) and larger Team (family/caregiver, internal and community-based services involved in providing care to the patient) in developing the patient driven holistic care plans. The RN Care Coordinator is a member of the PACT team. He/She is responsible for collaborating with services internal and external to the VA to facilitate transition of care that effectively meet the patients' needs. This includes but is not limited to chart reviews, telephone calls to patients for pre-visit preparation or post visit follow up, participation in team huddles, collaborative management of high risk patients, and making follow up telephone calls to patients discharged from the hospital. The RN Care Coordinator provides patient and family health education with a focus on self-management, prevention, and wellness, based on the patient's goals. The RN Care Coordinator demonstrates leadership by serving as an advocate for patients, team player to colleagues as she/he continues to enhance his/her own and the team's professional growth, development and practice. Job Duties include but are not limited to:

    - Assists in coordination of healthcare services and referrals.

    - Serves as patient advocate and direct liaison with physician and other healthcare providers, such as orthopedics, physical therapy, including unique VA healthcare system benefits.

    - Facilitation and coordination of non-VA care referrals and consults. 

    - Maintains awareness of patient's plan of care and communicates as necessary issues that arise, to prevent unnecessary delays in treatment, including missed appointments.

    - Facilitates acquisition of medical records and diagnostic testing to ensure timely delivery of services.

    - Assists with consults as per physician instructions in coordination with the referral team.

    - Active involvement in Cardiology clinics, participates in new consults with physicians and education as indicated for diagnostic procedures and treatment plan.

    - Assistance with medication management including tracking relevant laboratory values, monitoring treatment compliance, education on medications.

    - Assesses patients to identify needs, issues, and care goals in order to identify resources and critical factors for achieving desired outcomes of care.

    - Educates patient and/or family on specific needs that will facilitate their participation in the plan of care

    Work Schedule: Monday - Friday, 730am to 4pm
