    VA Connecticut Healthcare System was officially formed in 1995. We serve veterans in Connecticut and southern New England. The West Haven facility is a tertiary care facility classified as a Clinical Referral Level I Facility. It is a teaching hospital, providing a full range of patient care services, with state-of-the-art technology and a renowned research program. Comprehensive health care is provided through primary, acute, tertiary and long-term care in areas of medicine, surgery, psychiatry, physical medicine and rehabilitation, neurology, oncology, dentistry, geriatrics and extended care. Newington is an ambulatory care center that provides primary and specialty care. In addition to our main campuses, we operate six community-based primary care centers throughout the state.

    Responsibilities

    The Ambulatory Care Service Line at the VA Connecticut Healthcare Systems is recruiting for a full time Registered Nurse for Primary Care, Firm 2 at the Newington Campus. This is an exciting opportunity to work with the Primary Care Providers and other members of the healthcare team in delivering care for patients enrolled in primary care. The nurse will participate in patient education, preventive health inventions, management of acute and chronic disease states, telephone advice program, performance improvement activities, and care coordination/case management. The incumbent will be competent to triage and making decisions with the scope of the position. Documentation, data entry, data analysis and using data to manage outcomes are key elements of this assignment. The nurse will play an integral role in working with the clinic staff to develop processes that improve access to the clinic and clinical outcomes for the primary care patient population.

    Work Schedule: Monday through Friday, 8 am-4:30 pm must be able to work occasional Saturdays when necessary to provide care to the veteran population.

    Travel Required

    76% or greater - Must travel between Newington, Waterbury, Danbury, and Stamford Clinics.
