    The VA Salt Lake City Health Care System (VASLCHCS) is made up of compassionate employees committed to caring for Veterans whose sacrifices know no limit. We are part of the Rocky Mountain Veterans Integrated Service Network (VISN 19). Our health care system consists of the George E. Whalen Utah, Idaho, and Nevada. We provide health care to over 50,000 eligible Veterans across one of the largest geographical areas in VA. We are a teaching hospital, providing full range of patient care services, holistic medicine as well as education and research in partnership with the University of Utah and other institutions of higher education. Comprehensive health care is provided through our primary care providers and tertiary care is offered in areas of medicine, surgery, psychiatry, physical medicine and rehabilitation, neurology, oncology, dentistry, and geriatrics.

    Responsibilities

    The Nurse Physician Liaison (NPL) is a registered nurse with at least 5 years of clinical med/surg experience. He/she utilizes clinical expertise and program management skills to address the needs of acutely ill patients with complex medical, oncology, rehab, and mental health issues. The NPL functions as an integral part of the healthcare team, working in collaboration with doctors, nurses, social workers, and staff from other VA's and community services to facilitate patient care and discharge planning. He/she must demonstrate the ability to work effectively as a team member and must be able to gather and communicate crucial data in a manner that enhances patient outcomes within a dynamic, acute care environment.

    The NPL is assigned to inpatient Medicine and is accountable for the care and disposition of patients on the Acute Medicine and Telemetry Units. The duties of the NPL include the following: (1) using the nursing process to assess, plan, implement and evaluate the inpatient and discharge needs of complex patients, (2) documenting progress toward care goals for these patients, (3) educating the patients and family/significant others, (4) providing discharge planning and initiating referrals and/or consultations to achieve a multidisciplinary plan of care, (5) serving as a staff resource for patient care issues, (6) participating in hospital committees including the Skin Team, Bed Team and unit quality improvement efforts to meet regulatory standards and performance measures, (7) supporting administrative issues such as training and monitoring encounter completion by residents and inpatient attending physicians. Duties include but not limited to:

    • Acts as a liaison between physicians and nursing/ancillary staff.
    • Documents tentative plan of care and disposition for patients.
    • Documents discharge planning for in-patients on assigned medicine teams.
    • Documents Home Nursing/Hospice plans and referrals.
    • Plans for discharge/transfer of patients and completes discharge note in CPRS.
    • Disseminates a list of daily discharges with disposition plans to multidisciplinary discharge team.
    • Oversees the Discharge Expediter and communicates updates throughout day as to patient discharge needs.
    • Serves as a nurse/physician advocate.
    • Serves as an educator to inpatient physicians regarding CPRS and hospital policies and procedures.
    • Provides individualized patient care consistent with identified knowledge of patient's strengths and limitations.
    • Is proactive in identifying potential crisis situations and responds appropriately.
    • Initiates referrals and/or consultations as appropriate to achieve the multidisciplinary plan of care
    • Functions independently in performing more complex treatments, procedures and advanced technology specific to the scope of care on Acute Medicine and Telemetry.
    • Functions as a consult person to physicians, nursing and other personal for complex patient care issues.
    • Provides health care teaching to patient and significant others.
    • Functions as an inpatient case manager, insures that post hospitalization follow-up and discharge needs are completed prior to patient discharge.
    • Arranges for procedures, scheduling of diagnostic tests, and transportation home.Completes consults and forms for home oxygen and O2 flight orders.
    • Works closely with assigned physicians to determine best plan of care for patient.
    • Orders supplies for assigned patients (i.e. Wound care, PICC line, home infusion, and enteral feedings).
    • Collaborates with assigned Pharm D for Coumadin/Enoxaparin therapy follow-up.
    • Works closely with IT to coordinate the computer needs for the Medicine Service.
    • Assists with training and administrative orientation for house staff and faculty.
    • Serves as member of the Skin Team.Attends monthly meetings and functions as a Skin/Wound Champion for Acute Medicine and Telemetry.Provides bedside wound consultation and serves as a second verifier for suspected pressure ulcers.
    • Serves as member of the Bed Committee

    Work Schedule: Monday through Friday, 6:00 AM to 2:30 PM
