    The SFVAHCS is a teaching hospital providing a full range of patient care service, with state-of-the-art technology as well as education and research. Care is provided to eligible veterans in eastern South Dakota, northwestern Iowa, and southwestern Minnesota. Comprehensive health care is provided through primary care, tertiary care, and long-term care in the areas of medicine, surgery, psychiatry, physical medicine and rehabilitation, neurology, oncology, dentistry, geriatrics, and transitional care.     Responsibilities

    The Sioux Falls VA Health Care System is currently recruiting for Full-time, Day, Permanent, Registered Nurse - Patient Safety Manager. This position will be located in the Quality Safety Value Department. Nursing care is the dynamic and skilled application of a systematic body of knowledge to the diagnosis and treatment of actual or potential health problems. Competent nursing care is provided in the context of the nursing process, which encompasses all significant action taken by the nurse in providing care, and forms the foundation of clinical decision-making. Nursing care must be individualized to meet a particular patient's unique needs and situations. As a Registered Nurse - Patient Safety Manager, you are responsible for the following to include, but not limited to:

    • Investigates incident reports to identify causative factors and SAC scores events based upon actual and potential level of harm and frequency of occurrence. Then, based on findings and SAC scoring, implements appropriate process improvement models as needed.
    • Conducts literature searches and reviews for best practices.
    • Advises and facilitates Root Cause Analyses (RCA) and Aggregated Reviews when indicated and ensures that RCAs and Aggregates are completed in a timely manner with all technical and implementation requirements appropriately met.
    • Enters de-identified reports into SPOT assuring case correlation for program retrieval for all SAC 1 and 2 cases and selected sac 3 cases.
    • Facilitates and tracks the completion of patient safety alerts, advisories, and recalls.
    • Performs pre-construction risk assessments and construction rounds to proactively identify patient safety concerns.
    • Coordinates/Facilitates Healthcare Failure Mode Effect Analysis (HFMEA) reviews in accordance with Joint Commission standards.
    • Evaluates care areas for patient safety concerns during Environment of Care (EOC) Rounds throughout the organization.
    • Analyzes data, identifies trends and areas of concerns and elevates the information via daily interaction with leadership or via the composition and dissemination of periodic reports through appropriate committee structures.
    • Serves as a member on multiple organizational committees and collaborates with committee members on maintaining and developing system processes that promote patient safety.
    • Consults on workgroups and unit based meetings to provide support in the facilitation of safe patient care.
    • Provides follow-up on all actions and outcomes from process improvement activities including, but not limited to:
    • RCAs, Aggregated Reviews, Mental Health EOC Rounds, HFMEAs, and Patient Safety Assessment Tools (PSATs).
    • Provides education and training on patient safety topics to facility employees and introduces facility process related to patient safety at New Employee Orientation.
    • Analyzes and addresses facility needs based upon the results of the periodic Culture of Safety Survey.
    • Develops policies, procedures and standards at the program or service level related to patient safety.

    Preferred Experience:
    • Minimum 5 years of nursing experience
    • Experience in a healthcare leadership/management role or Experience in healthcare quality, patient safety or process improvement. 

    NOTE: Assignment to other areas of the service line, medical center, or other CBOC's may be required. The Registered Nurse reports to the Chief of Quality Safety Value, however, assignments may be made to other areas for purposes of cross coverage.

    Work Schedule: The tour of duty for this position is typically Monday- Friday. Flexibility in tours and assignments is required. Additional information regarding the work schedule of this position will be discussed during the interview process.
