    Responsibilities

    The Post 9/11 Transition and Care Management Program provides care management for severely ill and injury veterans of Operation Enduring Freedom (OEF), Operation Iraqi Freedom (OIF), and other modern hostile fire areas since September 11, 2001. It is aimed at providing and/or assisting in the access to medical and psychosocial services for Veterans once accepted into the program with access to all needed services within the Medical Center.

    The Nurse Case Manager is responsible for coordination of care management services for qualified Veterans receiving care within the Veterans Health Administration and those transitioning from medical care with the Department of Defense. The Program provides care management and clinical services provided by the Department of Veterans Affairs (VA) at its medical centers and in the community. Ongoing VA case management, healthcare, and other supportive services will be made available to qualified Veterans at as many as 132 VA Medical Center (VAMC) supportive services sites across the nation.

    The Post 9/11 Transition and Care Management RN works at the Kansas City VA Medical Center and in the community.

    Work Schedule: Monday - Friday, 8am to 4:30pm
