    VA Portland Health Care System (VAPORHCS) is a Level 1A major interdisciplinary teaching and tertiary facility with service-lines serving primary care, emergency medicine, medicine, surgery, specialty, psychiatry, physical medicine and rehabilitation, neurology, oncology, dentistry, geriatrics and extended care inpatients and outpatients throughout Oregon and Southwestern Washington. In addition to the two main divisions located in Portland, Oregon and Vancouver, Washington, VAPORHCS operates nine community-based outpatient / outreach clinics (CBOC) and is a consulting and referral center for many patient care specialties not offered at other VHA medical centers within VISN 20. There is a large outpatient psychiatric population, with admissions for medical co-morbidities. VAPORHCS operates a Community Resource and Referral Center (CRRC) which serves homeless Veterans in downtown Portland. The CRRC has a Homeless Patient Aligned Care Team (HPACT) co-located and offers primary care medical services to homeless and formerly homeless Veterans. Home-Based Primary Care nursing care is also provided throughout the VAPORHCS catchment area.

    VAPORHCS received initial Magnet designation in May 2006, re-designation in December 2010, and again in October 2015.

    The Registered Nurse (RN) for the Residential Rehabilitation Treatment Program (RRTP) practices in the VA Portland Health Care System (VAPORHCS) Mental Health and Neurosciences Division RRTP. S/He will work as a member of a multidisciplinary team providing inpatient services to veterans with a variety of substance use disorders and comorbid medical and mental health conditions. The primary duty station is located in the RRTP program at the Vancouver Division site.

    The RN for the RRTP functions as a team member under the guidance of the RRTP Nurse Manager and RRTP provider. Incumbent must be self-directed and able to function with minimal direction, communicating proactively and effectively with other members of the multidisciplinary team and external stakeholders. He/she is responsible for
    coordinating medical and behavioral care for RRTP residents for care both internal and external to the program, milieu management activities, oversight of RRTP LPN activities, completing screenings for Veterans requesting to admit to RRTP, and providing administrative support when needed. Demonstrates flexibility in taking assignments and completes a comprehensive system of tasks that are specific to RRTP. He/she works in partnership with the patient to create a treatment plan, monitors progress, and provides support for the patient as needed. The Incumbent must demonstrate the ability to observe, identify, and respond to the patient's needs for nursing care, including, but not limited to, patient education, medication administration, wound care treatments, EKG meter downloading, specimen collection including various urologic and breathalyzer procedures, suture and staple removal, capillary blood glucose testing, equipment assisted care, assessing for suicide risk, and recognizing deviations from normal including monitoring for signs of substance use and withdrawal. The Incumbent is also responsible for ensuring safety protocols including room checks and contraband searches are completed, conducting milieu management activities, and completing documentation. He/she performs age and culturally appropriate care, uses techniques to appropriately de-escalate disruptive patients, and appropriately participates in emergency response activities. He/she actively contributes to process improvement activities on the unit. Based on education and experience, may be assigned responsibilities as preceptors or charge nurse.
