If you are a US Citizen without status you should apply under Vacancy Announcement 654-17-AE-1996513-BU  The incumbent, a professional social worker, in this position functions as the Case Manager for VA Sierra Nevada Health Care System Mental Health Intensive Case Management Program. The primary responsibilities of this position are to provide intensive case management services, continuous quality improvement activities, evaluation/consultation, psychosocial assessments, patient groups, individual therapy, placement services, and referrals to various community agencies and hospital services to MHICM patients. MHICM patients are seriously mentally ill veterans with a high percentage of co morbid substance issues who have been identified as high utilizers of psychiatric inpatient hospital resources.  

MHICM patients have previously not been able to achieve adequate community adjustment with traditional psychiatric care. The patient caseload is complex based on the severity of the mental illness, co morbidities, and resistance to standard treatments. Treatment interventions are innovative and highly individualized. Patients are referred to the MHICM program from the outpatient psychiatry clinics, the inpatient psychiatric unit, and from outpatient medical and mental health providers.

Duties

Serves as the contact for the MHICM Program. Responsible for data collection, administrative record keeping, and monitoring patient or program indicators, thresholds and performance measures.

 

Responds to consults sent to the MHICM program by interviewing the veteran utilizing questionnaires and other measurement devices while assessing the veteran's emotional, functional, and psychological condition. Presents the findings to the MHICM team to determine rejection or acceptance into the MHICM program.

Functions as case manager for veterans in the MHICM program performing duties to facilitate independent community living by coordinating mental health and community care.

Participates in development of a treatment plan for each MHICM veteran with measurable goals outlined based on relevant psychological, medical, and psychiatric information with the principles of treatment matching.

Assists MHICM veterans in the preparation of materials needed for applications to various agencies for entitlements, then, follow the process of the application until a determination has been made.

Collaborates with members of the MHICM team and other professional specialty clinics to provide comprehensive care to MHICM veterans.

Identifies, develops and facilitates accessibility and utilization of VA and community resources to meet the needs of MHICM veterans.

Conducts individual and family clinical interventions to facilitate the identification of substance abuse, psychiatric illness, and social needs as treatment issues.

Assesses the MHICM veterans learning needs regarding their substance abuse, psychiatric and medical disorders, and the treatments and medications required incorporating teaching in the treatment.

  

Provides assistance with medication management for MHICM veterans.

Conducts home visits and community visits to MHICM veterans to maintain and encourage community living.

Collaborates with Adult Foster Care homes where MHICM veterans reside to facilitate community living in the least restrictive environment and to provide support services for the AFC homes.

Prepares and submit legal documentation for the petition of various legal orders such as guardianship, conservatorship, protective orders, commitment, and alternative treatment orders. Provides expert testimony in court settings as needed.

Provides direct teaching services to staff and others as appropriate.

Documents the veteran's progress in progress notes. Documents significant events as needed. Information is to be entered into the CPRS system.

Facilitates urine drug screenings, perform breathalyzer testing, and perform measurement of vital signs of veterans following MHICM procedures.

Responsible to stay abreast of current theory and treatment modalities by reading professional journals, attending conferences, clinic seminars and teleconferences, and materials given out by staff and Mental Health Service.
