    The VA Salt Lake City Health Care System (VASLCHCS) is made up of compassionate employees committed to caring for Veterans whose sacrifices know no limit. Wahlen Department of Veteran Affairs Medical Center in Salt Lake City, Utah, and nine community clinics across Utah, Idaho, and Nevada.     The Homeless Outreach Social Worker is responsible for providing clinical services at the VA Salt Lake City Health Care System (VASLCHCS), Homeless and Justice Clinical Recovery (HJCR) Program. With primary emphasis on assessment of patient needs, care coordination, facilitation of groups, individual psychotherapy and provision of psychoeducational classes. The social worker has good basic knowledge of Medical Center, VA and community resources needed to meet the needs of Veterans on the social worker's caseload. The social worker will also act as a resource consultant to members of both the VA interdisciplinary staff and broader Utah community partners.

    The incumbent's duties, include but are not limited to:

    • Provides outreach and crisis intervention services to homeless Veterans, seeking to address the cause as well as the presenting complaint.
    • Coordinates family conferences and serves as a liaison to family members.
    • Working knowledge and experience in the use of medical and mental health diagnoses, disabilities and treatment procedures, which include acute, chronic and traumatic illness/injuries, common medications and their effects/side-effects, and medical terminology.
    • Works closely with other VA Medical Center programs and services to ensure coordination between entities is facilitated, and direct care, referral, and follow-up for enrolled Veterans is contiguous and based on a continuum of services. Will act as a consultant to other staff/team members.
    • Possess knowledge and ability to independently implement treatment modalities, provide counseling or psychotherapy for individuals, families and groups. Incumbent independently conducts psychosocial assessments.
    • Develop treatment plans in collaboration with the Veteran/family and with the interdisciplinary treatment team and appropriate community partners. Possesses the skills to offer psychosocial interventions and measures outcomes to interventions.
    • Coordinate community resources and community-based services, including information and referral for additional services from other VA programs, other government programs and community agency programs. Incumbent establishes and maintains effective therapeutic relationships with homeless Veterans and, as appropriate, their families.
    • Coordinate and participate in community events including annual Homeless Veterans Stand Down.
    • Independently work with patients and families who are experiencing a variety of psychiatric, medical and social problems utilizing individual, group and family counseling and therapy skills.
    • Provide consultation services to other staff regarding the psychosocial needs of homeless Veterans and the impact of psychosocial problems on health care and adherence to the treatment plan.
    • Utilize the ability to evaluate his or her own practice through professional peer review, case conferences and other organizational means. Incumbent is able to communicate effectively, both orally and in writing, with people from varied backgrounds.
    • Utilize general knowledge of Veterans' benefits and services relating to special programs, service connected compensation and non-service connected pension.
    • Facilitate referrals based upon the Veterans' needs and eligibility.
    • Meet the needs of customers while supporting VA missions. Consistently communicates and treats customers (Veterans, their representatives, visitors, and all VA staff) in a courteous, tactful, and respectful manner. Provide customer with consistent information according to established policies and procedures.
    • Handles conflict and problems in dealing with the consumer constructively and appropriately.
    Work Schedule: Monday through Friday 8:00am to 4:30pm. May be required to work some weeknight and/or weekend hours in support of the program. To be negotiated with supervisor.
