    The VA Salt Lake City Health Care System (VASLCHCS) is made up of compassionate employees committed to caring for Veterans whose sacrifices know no limit. We are part of the Rocky Mountain Veterans Integrated Service Network (VISN 19). Our health care system consists of the George E. Wahlen Department of Veteran Affairs Medical Center in Salt Lake City, Utah, and nine community clinics across Utah, Idaho, and Nevada. We provide health care to over 48,000 eligible Veterans across one of the largest geographical areas in VA. We are a teaching hospital, providing a full range of patient care services, holistic medicine as well as education and research in partnership with the University of Utah and other institutions of higher education. Comprehensive health care is provided through our primary care providers and tertiary care is offered in areas of medicine, surgery, psychiatry, physical medicine and rehabilitation, neurology, oncology, dentistry, geriatrics, and extended care.

    This position falls under Nursing Services at the Salt Lake City Veterans Affairs Medical Center. The incumbent will be a professional clinician whose duties and responsibilities relate to the care management of Operation Iraqi Freedom (OIF), Operation Enduring Freedom (OEF), and/or Operation New Dawn (OND) service members and other Post 9/11 Veterans treated at the facility. The social worker must use a high level of skill in assessing and treating the complicated psychosocial problems of Post 9/11 service members and/or Veterans as they transition to Department of Veterans Affairs (VA) care.

    Care management responsibilities also include providing supportive services to families. In addition, the social worker assists Post 9/11 service members and/or Veterans in coping with acute illness, chronic illness, combat stress, the residual effects of traumatic brain injury (TBI), community adjustment, addictions, and other health and mental health problems. The social worker addresses home care needs, homelessness, and transition across levels and sites of care. Social Work care management practice, which includes psychosocial assessment, diagnosis, and treatment, is focused on helping Post 9/11 service members and/or Veterans and their families maximize rehabilitation and treatment potential and achieve more adequate, satisfying, and productive emotional and social functioning.

    Duties include, but are not limited to:

    • Uses the social work process (psychosocial assessment, diagnosis, and treatment) in collaboration with interdisciplinary team members to develop a care management plan and psychosocial interventions.
    • Evaluates the need for mental health services and makes appropriate referrals for individual, group, marital, and family treatment services.
    • Is responsible to interview patients, family members, and significant others to assess psychological needs, determine treatment, plan, and provide follow-up care.
    • Participates fully in developing, planning, implementing and evaluating the interdisciplinary treatment plan, including provision of care management services.
    • Determines appropriate level of care management and coordinates with VA Salt Lake City Health Care System / VA Mental Health Services and community agencies.
    • Coordinates care with an interdisciplinary team to promote continuity for Post 9/11 service members and/or Veterans and their families.
    • Develops and uses appropriate community resources.
    • Works with a wide range of complicated mental, emotional, behavioral, physical, psychosocial, and environmental problems. Maintains caseload as per Care Management guidelines and works toward Veteran independence.
    • Serves as an advocate for Post 9/11 service members and/or Veterans and their families, helping them access needed services at the facility, at other VA facilities, and in the community.
    • Maintains effective working relationships with intra-hospital/Primary Care/Mental Health Services, and community agencies to ensure continuity of care.
    • Assists Post 9/11 service members and/or Veterans and their families with advance directives, guardianships, and applications for home care and extended care services.
    • Participates in the orientation, training, and teaching of social work graduate students and other trainees and staff.
    • Provides consultation to other disciplines.
    • Conducts and/or participates in research and/or program evaluation as appropriate.
    • May serve on committees, work groups, and task forces at the facility, VISN, and/or VA Central Office levels.
    • Participates in interdisciplinary team meetings, appropriate facility meetings, and Social Work meetings. Shares knowledge and experiences gained from own clinical practice and education relevant to the field of Social work.

    Work Schedule: Monday thru Friday 8:00 am to 4:30 pm. (Possible compressed scheduled if approved by management)
