    Responsibilities

    The VA Maryland Health Care System (VAMHCS) Medicaid Eligibility Specialist Social Worker provides comprehensive clinical psychosocial and case management services to a diverse population of Veterans receiving care in the acute inpatient medical/surgical units.  Assists social work staff in the VAMHCS Inpatient Psychiatry and Community Living Centers and the Ambulatory and Emergency Care Clinical Center with targeted education and assistance to Veterans and their families regarding Medicaid applications.  Incumbent collaborates with the interdisciplinary team to meet biological, psychological, social, and environmental needs that impact on the ability of Veterans to make adjustments to health care issues.  Incumbent identifies, researches and documents vocational, medical and personal information for purposes of determining financial eligibility for Medicaid.  Serves as an advocate on behalf of the Veteran to ensure that eligibility for appropriate services/resources is evaluated and that referrals are made in a timely and consistent manner.  Incumbent completes Medicaid applications with input from the Veteran/family to aid in the discharge planning process.  Provides education/assistance to Veterans/families in the outpatient setting regarding Medicaid programming.  Assists other social workers in identifying and accessing community nursing homes, especially those with Medicaid-funded beds.  Provides intermittent cross-coverage to assist with inpatient medical/surgical social work functions during planned or unanticipated absences of regularly assigned social work staff and as assigned by Social Work supervisor.  Incumbent independently performs a variety of interventions in the amelioration of biopsychosocial stressors and is able to effectively perform interventions including high risk screening, psychosocial assessments, discharge planning, adjustment to illness counseling, end of life counseling, advance directives, resource education and referral, and coordination of care.  Participates in interdisciplinary rounds/meetings as needed based upon assignment.   Has a broad and comprehensive knowledge of VA and community resources and is able to coordinate community-based services, including information and referral for additional services from other VA programs, other government programs and community agency programs based upon Veterans needs and eligibility.  Incumbent has a basic knowledge of psychosocial treatment modalities and, under supervision, the ability to implement such treatment modalities in working with individuals, families, and groups to achieve treatment goals.  Incumbent serves as a liaison to community/state/federal agencies and network with agency representatives or, on occasion, perform visits/outreach to agencies with or on behalf of Veterans/families.

    Preferred Experience: Inpatient medical/surgical experience, including experience assisting with long-term care Medicaid applications is strongly preferred.

    Work Schedule: Monday - Friday 8:00 am - 4:30 pm. Some Saturdays may be required.
