    The SFVAHCS is a teaching hospital providing a full range of patient care service, with state-of-the-art technology as well as education and research. Care is provided to eligible Veterans in eastern South Dakota, northwestern Iowa, and southwestern Minnesota. Comprehensive health care is provided through primary care, tertiary care, and long-term care in the areas of medicine, surgery, psychiatry, physical medicine and rehabilitation, neurology, oncology, dentistry, geriatrics, and transitional care.     The Sioux Falls VA Health Care System is currently recruiting for one Full-time, Permanent, Staff Psychologist - Home Based Primary Care (HBPC), and Community Living Center (CLC). This position will be located in the Extended Care & Rehabilitation (EC&R) Service Line. As a Staff Psychologist, you are responsible for the following duties to include, but not limited to:

    Clinical Responsibilities: Functions as the primary mental health provider of the HBPC and CLC teams, providing mental health prevention, assessment, treatment, management, and professional consultation services. This individual will also provide coverage in other geriatrics and extended care settings such as the Hospice, Palliative Care and Polytrauma Teams. He/She will also serve as back up for the Pain Clinic Psychologist Program.

    • Provides a full range of psycho-diagnostic services including assessment and using the most appropriate psychotherapeutic techniques in providing quality care. Provides direct patient contact including neuropsychological assessment and treatment planning, individual psychotherapy/assessment sessions, family/caregiver stress therapy sessions utilizing best evidence base techniques.
    • Assessing and providing treatments for sub-clinical symptoms of dysphoria and anxiety, adjustment difficulties, & excessive use or misuse of alcohol or other drugs (for which the patient may have a lower than normal tolerance due to their medical conditions and the potential for negative interactions with prescribed medication);
    • Providing services to the families of Veterans with dementia and depression being served by HBPC and CLC, when such care is directly related to the overall treatment plan for the Veteran and will allow better care for the Veteran and allow the family to sustain the Veteran in the home environment.  For example, family members may be trained to better manage behavioral problems associated with dementia; such training may allow the CLC and HBPC team to reduce use of psychoactive medication as a means of controlling behavioral problems;
    • Providing screening/evaluations for patients with a variety of  psychological and neurological disorders including dementia, stroke, TBI, epilepsy, movement disorders, learning disability, ADHD, mood disorders, and personality disorders within the Poly trauma Team and Extended Care to address  specific functional questions or to determine if a referral for a complete neuropsychological evaluation or dementia assessment is needed;
    • Performing capacity/competency assessments when there are questions about a patient's ability to make medical decisions, perform other specific functions, or live independently;
    • Providing psycho-educational and supportive interventions, in conjunction with the Veteran, for spouses, family members/caregivers, and others that are crucial to keeping the Veteran at home;
    • Facilitating transition and adjustment to new living situations (e.g., nursing home, assisted living facility), and providing seamless intervention services, as appropriate;
    • Providing behavioral medicine interventions to manage pain, disability, address sleep problems, facilitate weight loss, promote smoking cessation, and enhance medical compliance;
    • Providing clinical supervision to students, interns, fellows, and residents.
    • Responsible to provide evaluation to patients with dementia related symptoms and diagnosis as part of the Geriatric Evaluation and Management Clinic.

    Administrative Duties:

    • Responsible for identification of patient problems requiring consultation with other multi-disciplinary medical center staff and making appropriate referrals, returning Veterans' phone calls.
    • Assures that appropriate referrals to specialty programs, including community resources, are accomplished.  Including ordering the following consults: Mental Health Programs, (i.e., Addictions, Post-traumatic stress, Mental Health Intensive Case Management, Compensated Work Therapy, Homeless, Day Treatment) Vocational Rehabilitation and Long Term Care.
    • Fully accountable for practice, including entering patient visits, measuring outcomes to interventions, and meeting productivity standards.

    Work Schedule: The tour of duty for this position is typically Monday - Friday, 8:00 am - 4:30 pm.  Additional information regarding the work schedule of this position will be discussed during the interview process.
